Private medical examinations and forms  are not services covered under our contract with the NHS and therefore attract charges. It is at the practice's discretion which forms we will do and they will usually be arranged outside of surgery times. Please talk with the reception team for an up-to-date list of charges.

Examples include the following:

• Power of Attorney interview and forms
• Insurance claim forms (eg mortgage applications, holiday cancellations)
• Private sick notes
• Pre-employment reports
• HGV driving medicals 
• DVLA reports or medical exam for normal driving licence requirements (DVLA pays fee)

We do not have expertise in assessing fitness to fly or sporting activities. These should be discussed with airlines or doctors who specialise in these fields. Letters from a GP are not usually required for medications to be taken on flights.
