Please could you look at any of the below sections that apply to you. If you need a copy of a form printed, please ask our reception team who will be able to provide this. 

• Asthma - Please could you complete the Asthma Control Test questionaire in the section below. 
• Chronic Obstructive Airways Disease - please could you complete the COPD Assessment test in the section below. 
• Diabetes - Please can you bring a first voided urine sample of the day to your appointment.
• Chronic Kidney Disease - Please can you bring a first voided urine sample of the day to your appointment.
• High Blood Pressure - Please can you bring a first voided urine sample of the day to your appointment and any recent blood pressure measurements.
